D.O.C. Identification: Ex-offenders should have this for
temporary use.
Birth Certificate is frequently required and should be obtained as
early as possible
ID Cards: take an original or certified copy of birth certificate,
passport, court order (document showing age verification) to offices below. Fill
out a form. Form is sent to Oklahoma City. In about two weeks an acceptance card
will be mailed back to you. Take acceptance card to a local tag agency which
will then issue the ID card.
Driver's License: requires 2 forms of identification. Primary
identification requires an original or certified document such as: Oklahoma ID
card, state issued certified birth certificate, city or county birth
certificate, passport, BIA ID card, Tribal ID cards, Oklahoma driver's license.
Secondary identification can be: Military discharge DD-214, original or
certified copy of marriage, separation or divorce judgment, Social Security
Card, Medicare/Medicaid card, health insurance card, Oklahoma student ID with
photo.
*NOTE: Picture ID's are essential for employment
